The Lake Cumberland District Health Department Environmental Health Services require a complaint form to be completed before an investigation is made.
The Environmental Department investigates nuisance complaints ranging from trash, garbage, surfacing sewage, rodent infestation and any other public health hazard.
It is best if nuisance complaints are made in person at the local health department during the Environmental office hours. If doing so creates a hardship, you may mail in a form or complete the web form below. If submitting the web form, all fields must be entered. If you don’t have enough information to complete each field, please come into the office during environmental office hours. Our locations and contact information can be found here: Locations

It should be noted that Nuisance Complaints are subject to the Kentucky Open Records Act.
